ASSESSMENT - a range of tools are used to understand a child’s strengths and challenges. This may include observation, tools to elicit a child’s views, dynamic assessment to identify areas of strength or challenge, and conversations with supporting adults

FORMULATION - Consultations and child-focused planning meetings give a shared understanding of the child and establish an inclusive action plan based on the knowledge and expertise of those that are closest to the child.

WRITTEN RECORDS - An efficient and meaningful record of involvement will be shared following the assessment.

REVIEW - A review is recommended after 6 - 10 weeks. The consultant can also support annual reviews.
